寻找抑郁症的终结者

目前对抑郁症治疗就像是扔飞镖，因为抗抑郁药还很不完美，有的要好几周才起效，有的对于很多患者完全没有效果，还有的会带来严重副作用，比如性欲减退。科学家正努力寻找更好的药物和疗法，K 粉、尼古丁受体和基因疗法等带来了新的启示和希望。

在“经验之谈”（Experience Project）网站上，一位网名叫做“蓝莓章鱼”的年轻女性讲述了自己使用抗抑郁药的经历。她服用抗抑郁药已经 3 年多，主要是为了抑制焦虑和恐慌。她最初服用的是帕罗西汀（Paxil），这是一种很常用的选择性 5—羟色胺再摄取抑制剂（selective serotonin reuptake inhibitors, SSRIs）。可是，一年后她不得不停止使用，因为这种药的副作用使她毫无性欲。于是，她换用另一种抗焦虑药物：赞安诺（Xanax），这虽然使她恢复了性欲，却对她的症状毫无帮助。之后，她又重新服用帕罗西汀，然后尝试了西酞普兰（Lexapro，另一种 SSRIs 药物），以及一种 5－羟色胺和去甲肾上腺素再摄取抑制剂（serotonin and norepinephrine reuptake inhibitor, SNRIs）——去甲文拉法辛（Pristiq）。在网站上发帖时，她正在服用左洛复 （Zoloft，另一种 SSRIs 药物）以及安非他酮 （Wellbutrin，与 SNRIs 很相似，可以同时影响多巴胺和去甲肾上腺素的活性）。服用安非他酮是为了“中和”左洛复的副作用——性欲减退。“我并没有感到服用安非他酮以后有什么不同，不过我现在使用的是最低剂量”，她写道，“下周我要再去看我的心理医生，也许他会给我加大剂量。谁知道呢”。

这样开处方，是一种典型的试错法（trial-and-error）。除了治疗抑郁症，这种方式也被用于治疗其他类似的精神疾病。安德鲁·所罗门（Andrew Solomon）曾在他的代表作《忧郁》（The Noonday Demon）一书中写道，这种试错的方式“使你觉得自己就像一个飞镖盘”。

让人伤脑筋的副作用并不是导致“飞镖盘疗法”的唯一原因。自从 SSRIs 和 SNRIs 于 19 世纪 80 年代和 90 年代被用于治疗以来，就一直是抗抑郁药市场的主打产品，但它们并不能帮助每一位患者——超过三分之一的患者服用后完全没有作用。今天有效的一种药物，明天可能就失效。有时候，一种药物要等好几周才开始见效，而在等待的几周里，患者可能非常危险。根据 2006 年《美国精神病学杂志》（American Journal of Psychiatry）上的一份报告，使用 SSRIs 的老年（66 岁及以上）抑郁症患者，第一个月内自杀的风险是第二个月的 5 倍。

显然，患者急需更快更好的抗抑郁药，可是新药研发却停滞不前。事实上，在过去几年，像葛兰素史克（GlaxoSmithKline）这样的制药巨头都发表声明，打算放弃精神疾病药物的研发，原因就是这些药物的研发成本太高，太艰难，成功率太低。

一些在政府部门、科研机构以及小制药公司的科学家，正在试图解决这个难题，但是他们能否成功还不得而知。不过，目前不可能很快研发出新药，来满足美国大约 1 500 万抑郁症患者。很多患者还得不到治疗。他们迫切希望尝试任何可能的方法，来缓解精神痛苦，甚至愿意尝试一些极端的实验方法，比如把电极插入脑中，或者在脑组织里烧洞。

寻求快速见效

一些研究人员为了寻找快速见效的抗抑郁药，正在研究一些可以即刻起效的情绪调节剂，例如氯胺酮（Ketamin）。他们希望弄清楚，为什么这些化合物能比 SSRIs 更快地发挥作用。

氯胺酮是一种麻醉药，能镇痛，并能带来快感，俗称 K 粉（Special K）。它能够影响人们的意识，使人产生幻觉。动物实验显示，这种药物会毒害神经细胞，因此并不是一种理想的抗抑郁药。不过，对于研究如何才能快速缓解抑郁症患者的症状，这是一种很棒的药物。美国耶鲁大学的罗纳德·杜曼（Ronald Duman）和乔治·阿加贾尼扬（George Aghajanian）等人发现，给大鼠注射氯胺酮仅两小时后，它们的前额叶皮层（prefrontal cortex）就会产生形成新突触所需要的蛋白（突触是神经细胞间传递信号的接触点）。而在很多抑郁症患者中，这个位于眼睛正后方的大脑区域都出现了异常。注射氯胺酮 24 小时后，大鼠神经细胞的树突上开始出现新的树突棘——这些突起是用来接受其他神经细胞发来的信号。树突棘越多，神经细胞间的信号传导就越快。杜曼和阿加贾尼扬在实验中发现，树突棘越多，动物抑郁症状（比如不会参加在正常状态下本会参与的活动）就越轻微。

“近十年，很多研究显示，处于抑郁状态时，前额叶皮层和海马区（hippocampus）都会萎缩”，现为耶鲁大学分子精神病学实验室负责人的杜曼介绍道，“氯胺酮可以快速逆转萎缩”，使前额叶皮层和海马区恢复正常。氯胺酮的见效速度究竟有多快，正是耶鲁大学科学家的研究课题。他们给大鼠注射氯胺酮几小时后，就开始研究大鼠大脑，看新的树突棘是否在 24 小时内就会形成。

用抑郁大鼠进行的另一项研究，已经揭示了氯胺酮促进突触形成的机理：通过激活神经元中一种叫做 mTOR 的酶。这一机理，是杜曼及其同事使用了 mTOR 抑制剂后发现的。他们先给大鼠使用抑制 mTOR 的药物，然后注射氯胺酮。结果发现，这些大鼠既没有形成新的树突棘，抑郁症状也没有减轻。这意味着，当 mTOR 被抑制时，氯胺酮无效了。换句话说，氯胺酮需要通过 mTOR，才能发挥促进树突棘形成的作用。

鉴于氯胺酮作为药物使用风险太大，科学家开始寻找其他的 mTOR 激活剂。他们知道，氯胺酮激活 mTOR 是通过阻止谷氨酸（glutamate，大脑中最重要的一种兴奋性神经递质）与神经元表面 N- 甲基 -D- 天门冬氨酸（NMDA）受体的结合来实现的。因此，他们测试了另一种 NMDA 阻断剂，结果发现这种 NMDA 阻断剂也能激活 mTOR，并能快速促进树突棘形成和减轻大鼠的抑郁症状。杜曼说，目前他们正在研究其他一些能够阻断 NMDA 受体的药物，希望从中找到安全、见效快速的抗抑郁药物。

另外一种像氯胺酮一样、能够快速舒缓情绪的药物已经因为其他功效上市了。这种名叫东莨菪碱（scopolamine）的药物，通常被制成膏药销售，用来治疗各种晕动病（motion sickness）。东莨菪碱对大脑回路的影响与氯胺酮不同：它会阻止乙酰胆碱（acetylcholine，一种与注意力和记忆有关的神经递质）与毒蕈碱受体（muscarinic receptor，一类广泛分布在大脑中的乙酰胆碱受体）的结合。

早在 19 世纪 70 年代，研究者们就注意到，人为改变大脑中乙酰胆碱的活性可能会导致抑郁。双相情感障碍患者（bipolar patients）的情绪通常在狂躁和抑郁之间来回转变。当他们表现得狂躁时，如果服用会提高乙酰胆碱水平的药物，他们在一个小时内可能就会表现出抑郁症状，例如心情悲伤、无精打采。而当抑郁症患者服用能够提高乙酰胆碱水平的药物时，他们的抑郁症状便会恶化。

看到这里，你也许在想，研制新型抗抑郁药的研究者只要找到方法，阻止乙酰胆碱发挥作用就行了。不过，在那个时代，研究者的注意力被当时最受关注的神经递质——5－羟色胺给分散了。事实上，很多精神病医生都曾认为，SSRIs 之所以如此有效，就是因为它们不会影响大脑中通过乙酰胆碱发挥作用的神经回路。于是，乙酰胆碱没有受到重视，因为医生们认为，先前那些抗抑郁药之所以不如 SSRIs 有效，有很多副作用，就是因为它们会影响胆碱系统，特别是毒蕈碱受体。

因此，如果有人说，他找到了一种专门作用于毒蕈碱受体的药物，不仅副作用少，而且见效快，这根本就是在挑战传统思维。不过，一些科学家利用东莨菪碱达到了这样的效果。

在一个有 22 名患者参与的临床实验中，美国国家心理健康研究所（National Institute of Mental Health, NIMH）实验治疗学与病理生理学分所的莫拉·弗雷（Maura Furey）等人发现，静脉注射东莨菪碱能在三天之内缓解抑郁症状。弗雷说，事实上，第二天早上起来，患者基本上就感到病情好转。在为期四周的实验结束时，近三分之二的患者明显好转，而且一半患者的症状完全消失。最后一次用药后，这种良好的状况可以持续两周。而在另一个也有 22 名患者参与的实验中，研究人员得到了同样的结果。

NIMH 希望找到一家制药公司，进行必要的测试和临床实验，以便将东莨菪碱作为一种速效抗抑郁药推向市场。目前还没有制药公司愿意接手，弗雷对此表示“非常失望”，她说，“这种药在患者身上真的很有效”。

东莨菪碱的给药途径是一个绊脚石。像一些麻醉师注射麻醉合剂那样，通过静脉注射东莨菪碱是不现实的。而做成皮肤膏药，血液中的药物浓度没法达到理想水平。作为口服药，大部分东莨菪碱会被消化系统清除。弗雷现在正在致力于寻找一种既可行又有效的给药途径。

关注无效人群

除了要经过很长时间才能起作用之外，现有抗抑郁药的另一个重大缺陷是，它们并不能对每一个人都有效。为了解决这个问题，科学家正在集中研究几种新的作用机制。一些人在研究另一类乙酰胆碱受体——尼古丁受体（nicotinic receptor），这类受体因能与尼古丁结合而得名。美国一家小型制药公司 Targacept 的科学家正在研究一种名叫 TC－5214 的实验性药物，它能阻断一种特殊的尼古丁受体。科学家希望将这种药物作为辅助性药物推向市场，当单独使用一种药物不能减轻症状时，他们希望添加这种药物能够有所帮助。

在一项有 265 人参与的初期实验中，单独服用西酞普兰（Citalopram）这种 SSRIs 药物没有效果的患者，补充服用了 TC－5214 或者安慰剂。2009 年，Targacept 公司发布报告指出，那些同时服用西酞普兰和安慰剂的患者，在汉密尔顿抑郁量表（Hamilton Rating Scale for Depression）上的得分提高了 7.75 分，而那些同时服用西酞普兰和 TC－5214 的患者则提高了 13.75 分。

不久之后，阿斯利康（AstraZeneca）公司与 Targacept 公司签署了合同，准备进行更大规模的疗效研究——II 期临床实验。在 II 期临床实验中，受试者除了服用原抗抑郁药外，还会服用安慰剂，或 TC－5214。614 人参与了前两次实验，得到的结果令人失望：与安慰剂组相比，TC－5214 组在 8 周后的抑郁症状没有任何好转。但 Targacept 公司和阿斯利康公司仍然决定，按原计划再进行两次药物疗效实验，将在遍布全球的多个测试中心完成，受试者将超过 1 300 人，新的临床实验还将测试药物的长期安全性。他们期待在 2012 年下半年，向美国食品及药品管理局（FDA）提交 TC－5214 的新药申请。

由于 TC－5214 是一种尼古丁受体拮抗剂（antagonist），它的作用机理不是影响 5－羟色胺或去甲肾上腺素，因此这种药物主要帮助那些服用现有药物没有效果的抑郁症患者。另外一种针对这些人群的方法，会带来更彻底的改变：不是通过受体来影响信号通路，而是作用于另外一种生物过程——神经再生（neurogenesis，新神经元的生成和生长），尤其是在海马区内。海马区是大脑底部的一个小结构。一般认为，成年人大脑内神经再生主要发生在两个部位，海马区就是其中之一。

很早以前，人们就认为海马区结构的改变与抑郁有关。对抑郁症患者大脑的尸检显示，抑郁症患者的海马区萎缩，体积明显变小。SSRIs 以及 SNRIs 药物之所以能缓解抑郁症，除了它们能够调节 5－羟色胺的浓度，还在于它们能促进新海马细胞的生长。然而，这一生长过程很缓慢，这也许就是上述药物要很长时间才能发挥作用的原因。美国一家小型制药公司 Neuralstem 的科学家认为，他们已经发现了另外一种方法，能促进神经再生，而且在停止用药后仍然能维持神经再生。

为了寻找新药，Neuralstem 公司的科学家利用了体外培养的、由人类海马细胞得到的神经干细胞。据该公司称，只有他们在做这种体外培养。根据药物对体外培养的海马细胞的作用情况，科学家首先筛选了大约 10 000 种药物。该公司首席科学家卡尔·乔（Karl Johe）说，他们的目的是看七天后，哪些药物能提高细胞增殖率。他说，只有不到 200 种化合物达到这个要求。在此基础上，Neuralstem 公司的研究团队设计了 12 种很可能促使海马区神经再生的化学分子。2004 年，研究人员开始进行动物实验，将这些备选的药物注射到正常小鼠体内。然后，挑选出效果最好的药物，注射到具有抑郁症状的小鼠体内。经过这样的实验，最有希望的那种药物浮出水面了。

现在，Neuralstem 公司正在进行初期的安全性测试（I 期临床实验），检测这种被命名为 NSI－189 的药物片剂对人类是否安全。如果进展顺利，Neuralstem 公司希望在 2012 年年底开始进行药物疗效的测试，比如用磁共振成像来检测药物能否促进神经再生，以及通过其他一些测试，来检测抑郁症状能否得到缓解。即使 NSI－189 有用，也不大可能在短时间内见效。“不像癫痫患者发作时，你给他一片药就能止住”，乔解释说，“这种疗法需要从基因层面改变细胞”。海马区萎缩是经过多年才发生的，“那么逆转这个过程也需要相当长的时间”。不过，他希望效果能长期持续，这样就只须间歇性地服用 NSI-189。这个想法还有待证明，但是乔说，“这种可能性令人兴奋”。

更深入研究

研究发现，慢性炎症（chronic inflammation）和许多疾病有关，比如癌症、动脉粥样硬化和糖尿病。最近，科学家已经意识到抑郁症也与慢性炎症有关，这种联系已经开辟了全新的治疗途径。

炎症通常被认为是身体对异物入侵的反应。多项研究结果显示，炎症和抑郁症之间存在某些联系。一些研究表明，抑郁症患者血液中有高浓度的细胞因子（cytokine），这些细胞因子是协调炎症过程的小分子蛋白，比如白细胞介素 6（interleukin-6）和肿瘤坏死因子α（TNF-α）。此外，大约十年前，科学家观察到，皮肤癌患者接受炎性细胞因子治疗后会变得抑郁。

美国埃默里大学温希普癌症研究所精神肿瘤科主任安德鲁·米勒（Andrew Miller）说：“早些年，我采访过一个这样的癌症患者，他的抑郁表现，和我作为心理医生遇到的抑郁症状极其相似，我感到非常惊讶。”

细胞因子有一点特别不好，它们会干扰 SSRIs 和 SNRIs 促进神经再生的过程。“破坏神经再生几乎就是对抗抑郁药釜底抽薪”，米勒说。这种效应有助于解释，为什么那些慢性炎症非常严重的抑郁症患者最难治愈。2006 年，科学家在《柳叶刀》（Lancet）杂志上报道，在 618 例患者中测试依那西普（Etanercept）对于银屑病（psoriasis）的疗效时发现，依那西普能减轻抑郁，甚至在对银屑病无效的病人中也是这样。看起来，这似乎是因为依那西普对炎性分子——肿瘤坏死因子α有中和作用。研究小组的一名成员、美国杜克大学的郎加·克里希南（Ranga Krishnan）说，“目前来说，患者还不能要求医生开这种药来治疗抑郁”，依那西普抗抑郁的结果目前还只能算是一个趣闻，“不过，这种结果着实令人激动”。

米勒也认为这项研究非常有意思，他联系上克里希南，讨论了如何利用一种细胞因子拮抗剂——英夫利昔单抗（Remicade），来开展一项抗抑郁实验。英夫利昔单抗是一种已经上市的消炎药物，用于治疗类风湿关节炎（rheumatoid arthritis）和其他自身免疫疾病（autoimmune disease）。历时五年多，米勒和他在埃默里大学的同事查尔斯·拉伊森（Charles Raison）终于拿到 NIMH 提供的、用于开展实验的经费。他们已经用英夫利昔单抗，对 60 名普通疗法无效的抑郁症患者进行了实验，很快将公布一些可喜的成果。

一些研究人员正在把目光重新投向 5－羟色胺，不过他们正在尝试一种新方法，通过增加突触表面 5－羟色胺受体的数量，来提高这种神经递质的活性。还有科学家提出了更为激进的方案，打算通过基因疗法来实现这个目标。

在生物学家面前提起基因疗法，你很可能会看到他们不屑一顾的眼神。不过，最近科学家宣布，对一名帕金森氏病（Parkinson’s disease）患者进行基因治疗取得了初步成功。参与这次帕金森病治疗研究的一名科学家希望用类似的方法治疗抑郁症。

基因疗法所针对的候选基因叫做 p11，某些 5－羟色胺受体迁移到细胞表面时，需要该基因编码的蛋白（p11 蛋白）。如果没有 p11，5－羟色胺受体就会被困在细胞内，细胞对 5－羟色胺信号的响应就会减弱。2006 年，美国洛克菲勒大学的保罗·格林加德（Paul Greengard）等证明，具有抑郁症状（比如不再做以前喜欢的活动）的老鼠中，p11 蛋白很少。此前对人的尸检显示，抑郁症患者中 p11 蛋白低于正常水平。

格林加德的实验室通过基因敲除，培育出了缺乏 p11 基因的小鼠，这些小鼠确实表现出了抑郁症状。这项研究的下一步工作，是由美国康奈尔大学威尔医学院分子神经外科实验室的负责人迈克尔·卡普利特（Michael Kaplitt）和同事完成：重新向这些小鼠插入功能完好的 p11 基因，看它们的症状会不会减轻。卡普利特已经在用基因疗法治疗帕金森病。他也是使用不能复制的腺病毒（defanged adno-associated virus），将 p11 基因直接运送到缺乏 p11 基因的小鼠的伏隔核（nucleus accumbens）中。结果，这些小鼠的抑郁症状减轻了。

每一位神经科学家都有自己最喜爱的大脑区域，卡普利特钟爱的是伏隔核。他说：“我喜欢伏隔核，因为它是大脑中一个非常重要的中心，负责奖赏和满足感，这是多巴胺（dopamine，一种神经递质）的作用区域。”卡普利特说，有一种常见的抑郁症状叫做快感缺乏（anhedonia），患者根本体会不到生活的快乐，这种破坏性非常严重的症状很可能与多巴胺信号通路有关。他喜欢伏隔核还有一个原因：对动物和人进行的功能性磁共振成像（fMRI）研究表明，许多与抑郁症有关的脑区，都和伏隔核有神经连接。

他喜欢伏隔核的第三个原因是，伏隔核已经成为另一种实验性抑郁症疗法的手术目标，这种技术被称为“大脑深部刺激”（deep-brain stimulation, DBS）。该疗法是在伏隔核中永久性地植入电极，然后通过电极，定期进行电脉冲刺激。

在卡普利特看来，直接对大脑施行基因疗法比 DBS 更简单，因为“DBS 需要一个电极，而基因疗法只需要一根小导管，而且不会在大脑中留下任何东西”（DBS 不仅需要永久性地在大脑中放置电极，而且需要在锁骨处植入类似心脏起搏器的、用来产生电脉冲的神经刺激器）。卡普利特等人在研究帕金森病疗法时，已经证明了病毒载体是安全的，而且通过一根导管，就可以把正确的基因输送到目标脑区，从而改善症状。

在 NIMH，神经心理学实验室的伊丽莎白·A·穆雷（Elisabeth A. Murray）和灵长类动物实验中心的帕姆·诺布尔（Pam Noble）正在测试 p11 基因疗法在猴子身上的安全性和疗效。如果实验成功，将为这种基因疗法的人体实验奠定基础。

对于“蓝莓章鱼”而言，更好的治疗方法并不会很快出现。“抗抑郁药的确改变了我的生活”，她在“经验之谈”网站上写道，“但是，让我感到遗憾的是，我付出了很大的代价，我的性生活受到了影响”。她?? 还不到 25 岁。“最后，我可能会放弃抗抑郁药，恢复正常的性生活。我只是觉得现在还没准备好”。生活本应该有更好的选择，不应该让任何人面临性和绝望的抉择。对于任何抑郁症患者，即使尝试过许多种疗法都遭到失败，我们也不该告诉他们希望渺茫。如果新一代抗抑郁药研发成功，也许使用时的利弊权衡就不会那么残酷。
